Medicare Advantage (MA) has long had coverage for home health patients, but there has never been coverage for Hospice under the MA benefit. VBID is a demonstration that began January 1, 2021 and continues for 5 years. There are multiple Medicare Advantage Organizations (MAO) that are participating and have coverage in multiple states. In addition to Hospice care there is an expansion of Palliative Care services that are offered to patients through the MAO. CMS is testing how the hospice benefit component can improve beneficiary care through greater care coordination, reduced fragmentation, and transparency in line with recommendations by the Office of Inspector General (OIG), the Medicare Payment Advisory Commission (MedPAC) and others. Hospice must prepare as more and more MAOs are participating in VBID to offer the hospice benefit to their beneficiaries.Read More

To ensure optimal care, a pharmacist led interdisciplinary team conducted a quality improvement project at a non-profit home healthcare agency focusing on improving medication reconciliation and evaluation in patients at high risk for hospitalization. The project connected an improvement in medication reconciliation to a reduction in 30-day hospitalizations in patients with heart failure. Prior to the project, the baseline median for 30-day hospitalizations was 20.0%; following full implementation, this dropped 9.1% among patients with heart failure. This presentation describes the development of the interdisciplinary quality improvement team, the approach of the team, the selection of the metrics, the overall results, and future directions for further collaboration between home healthcare agencies and pharmacists.Read More

While over 30% of patients in home health have a wound, providing standardized, best practice care is a challenge. In 2020, a home health agency that provides services across five states, launched a nurse-led, remote-enabled wound care program with their innovative technology partner. To underpin this new, virtual wound care program, the nursing team adopted an AI-powered, digital wound care platform to allow frontline clinicians to easily capture high precision wound images in the home and then share this data with the remote wound experts to augment clinical decision making to drive more preventive care.
